The Delaware HIV Consortium provides Ryan White HIV Case Management services under contract with the Delaware Division of Public Health. For clients who qualify for Ryan White benefits, the Consortium offers HIV Case Management and Ryan White Support Services statewide to help you manage your HIV. HIV Case Management services include:
- HIV Case Management, including personal support services and referral to resources to help you better manage your HIV
- Access to medication
- Access to HIV housing assistance
- Access to Emergency Financial Assistance
- Access to Health Insurance Assistance
- Access to dental services
- Access to vision services
- Access to mental health services/counseling
- Access to transportation assistance
- Access to food assistance
