Services
Our HIV Care Services are designed to support people living with HIV in achieving health, stability, and independence. Through compassionate, coordinated case management, we connect clients to medical care, housing assistance, mental health counseling, food and hygiene access, and financial support. Each client receives individualized guidance to overcome barriers to treatment, adhere to medication, and access community resources that improve quality of life. From intake to ongoing engagement, our team ensures that every person receives care that reflects their unique needs and lived experiences.
We deliver these services through a client-centered model that integrates medical, behavioral, and social supports. By working collaboratively with healthcare providers, housing agencies, and partner organizations, we promote continuity of care and equitable access to essential services. This approach not only helps clients achieve long-term wellness but also strengthens the overall health of our community ensuring that every Delawarean living with HIV can thrive with dignity and support.
