Case management provides a range of client-centered services that link clients with health care, psychosocial and other services. Case management ensures the timely and coordinated access to medically appropriate levels of health and support services and continuity of care, through ongoing assessment of the client’s and other key family members’ needs and personal support systems. It also seeks to increase client independence and self-sufficiency, while working to prevent unnecessary hospitalizations or to expedite discharge from an inpatient facility. It also promotes coordination among service providers and other support systems to minimize the duplication of resources.
The Delaware Consortium initiated its Ryan White Case Management program in April of 2012, in order to provide a safety net for clients during a major transition of current case management providers. On April 1, 2012, all current case management service organizations were contracted with managed care companies to dually coordinate the Medicaid AIDS waiver case management system for a 15-month period. In order to ensure that no client was lost to care, and to prevent waiting lists, the Delaware HIV Consortium began providing Case Management services only to eligible Ryan White clients in New Castle County. As the funding landscape and case management model changes during this transition period the Delaware HIV Consortium is poised to fill the gaps that may occur in case management services.
