| Case Management | |||||||||||||||||||
This category provides a range of client-centered services that link clients with health care, psychosocial and other services. Case management ensures timely and coordinated access to medically appropriate levels of health and support services and continuity of care, through ongoing assessment of the client's and other key family members' needs and personal support systems. The community-based programs seek to increase client independence and self-sufficiency, while working to prevent unnecessary hospitalizations or that expedite discharge from an inpatient facility. The programs also promote coordination among service providers and other support systems to minimize the duplication of resources.

| Emergency Financial Assistance | |||||||||||||||||||
This program provides short-term payments to agencies to assist with emergency expenses related to critical needs with housing (rent and utilities) when no other means are available. In special circumstances, this program may also pay for emergency shelter for HIV infected persons who are in danger of being homeless.

This program provides conveyance services for clients so that they may access health care or support services.

NON-CONTRACTED SERVICES |

| Mental Wellness & Nutritional Counseling | |||||||||||||||||||
Within a fee-for-service payment structure, this program uses licensed providers to offer various private, community-based services to eligible persons with HIV disease. These services include: (1) mental wellness counseling: psychological and psychiatric treatment and counseling services to individuals with a diagnosed mental illness, usually in an one-on-one setting. Assessment is included as a major component of the service; and (2) nutritional counseling: nutrition education and/or counseling provided by a licensed dietician outside of a primary care visit. Referrals for this service generally are made by the client's primary care physician or through the HIV Wellness Clinics. Emphasis is placed on addressing the special dietary needs of persons living with HIV disease.

| Health Insurance Continuation | |||||||||||||||||||
This program provides financial assistance to maintain continued health insurance coverage for eligible individuals living with HIV disease who are in danger of losing their insurance coverage due to their inability to pay.

| Short-Term Housing Assistance | |||||||||||||||||||
This program provides rental assistance for eligible clients who are currently on the waiting list for the Consortium's Housing Opportunities for Persons with AIDS (HOPWA) funded rental assistance program. Providing financial assistance to clients for no more than 18 months, the program is a supplement to the Consortium's rental assistance program. The goal of the program will be to foster stable living situations to optimize the outcomes of consistent medical care in the treatment of their HIV disease.
